We at Life Rythm provide counseling sessions for varied mental health issues, we however do not provide assessments and diagnosis. Once a client is registered with us we assume that the following terms and conditions have been read and understood. The booking and confirmation of the session imply that the client has read agrees to the above terms of service. ²
Clients will be well informed at least 24 hours before the session depending on the availability of both the clients and the Counselor.
- A thorough consent is taken before the beginning of the session ² Payment is to be done well in advance, post-booking the session
- The duration of the session is 45 minutes, clients who come in late for the appointments will not be given extra time rather can only avail themselves of the remaining time of the session. The payment however will remain the same.
- Cancellation Policy- If any client wishes to cancel the session, it should be well informed at least 24 hours before. Any cancellation after that will not be considered and would be amounting to fees for the pre-decided session.
- We understand the implications of Suicide or self-harm and regard these as tragic events which bring great distress to the person's family and relatives. However suicidal tendencies or harming one-self are acts that are considered to be impulsive in nature and Life Rythm cannot be held liable for this. For any such acts, emergency and intervention centers and services must be contacted.
- We preach confidentiality, although cases are discussed with other qualified and senior mentors with undisclosed identities.
- In cases, of Online sessions, they are subject to technical limitations hence formal channel will be pre-decided keeping in mind the logistics on both ends.
- In case of technical issues during the online session, these will be only considered for the first session assuming that in the latter online sessions problems are going to be tackled by the client.
- Initial Session: this is your first booking where you meet your therapist to decide whether you could do good work together. This initial meeting aims at clarifying your needs and the best way to move forward, including frequency of sessions, length of therapy, and possible referrals to other professionals.
-The first consultation with the client is will not be counted from the number of sessions mentioned in the monthly plan. The first consultation will be 20-30 minutes and would be not chargeable.
- Sessions confirmation: sessions are confirmed for the 24 hours prior and any changes should be conveyed well in advance.
- Fees once paid are not refundable, however, are adjustable in the form of rescheduling of the session.
All the information shared before, during, and after the therapeutic process is bound by the client-therapist contract at Life Rythm. The information is secure, confidential, and protected with regards to our terms and conditions discussed with the client before commencement of therapy.
Below is a detailed record of the information we collect, how it's used, what is shared, its safety and security, and your rights in regards to the same.
Clause 1. Information we collect:
Initial Assessment – At an initial appointment we ask about your current personal, social, medical, and financial circumstances. We may also ask about your background and family history, as well as the issues which are affecting you now. We require this information so that we can decide about our offer of counseling to you, to assign you to a counselor, and to manage the service we provide to you.
Other Forms – The information you give us on our forms (including all inquiry and application forms) may include your name, postal address, email address, phone number, and other messages to us.
Clause 2. What do we use your information for?
· To provide clients with the professional counseling service requested from us.
· To enable us to offer appropriate opportunities and support to our counselors, receptionists, and other volunteers.
· To offer suitable counseling appointments, and to allocate clients and counselors for counseling.
· To notify you about changes to your appointments and other changes to our services.
· To seek feedback from you on your experience of counseling with us.
· To improve our service to ensure that it is provided most effectively for you and us.
· To administer our service, including the arrangement of appointments, the handling of donations, and for financial control, data analysis, research, statistical, and survey purposes.
· To keep in touch with those who consent to this, for organizational, service, and professional development.
· To fulfill our administrative, legal, and contractual obligations as an employer.
Clause 3. What information do we share?
We will not share any information about you with other organizations or people, except in the following situations:
· Consent – We may share your information with professional carers or others whom you have requested or agreed we should contact.
· Remote Appointments – We may temporarily share your contact details with counselors who conduct appointments remotely, so that they can contact you with regard to those appointments.
· Serious harm – We may share your information with the relevant authorities if we have reason to believe that this may prevent serious harm being caused to you or another person.
· Compliance with law – We may share your information where we are required to by law or by the regulations and other rules to which we are subject.
Clause 4. How we keep the information safe?
-All information you provide to us is stored as securely as possible. All paper forms and correspondence are kept in locked filing cabinets on our premises.
-Your identifiable personal information is kept separately from any session notes and other descriptive material. Client notes and other documentation are destroyed 3 years after the end of counseling.
Clause 5. Your rights
You have the right to ask us to provide a copy of the information held by us in our records. You also have the right to require us to correct any inaccuracies in your information. If you would like to do this, please contact us at info.liferythm@gmail.com
Clause 6. Changes to this policy
We may edit this policy from time to time. If we make any substantial changes we will notify you by posting a prominent announcement on our website.
